Gee... That guy as the cursor looks so familiar, but I've never heard of Cave Story.
 
The last form of Ballos is easy. You just jump into his eye and blast the fuck out if it with the rocket launcher. It's an instant kill. Try it. It's fun. x3

I actually tried that, but fell short so instead I hovered below him firing rockets. Still only took me a few seconds more.
