1. The ability to transform into anything. This would technically also solve aging, since when I'd get older, I could just transform into a younger version of myself.
2. Nothing bad to ever happen to me. This would solve the problem of death assuming that it's considered as a bad thing from the point of view of the one who would make these wishes come true (that one could think that death is a good thing so that the Earth doesn't get overpopulated etc. or something, then again if only I got the option to wish, then 1 person not dying wouldn't be a problem, and after all I didn't wish that nothing bad would ever happen to my family, because in that case it would be assumed that none of them would die in case death is viewed as a bad thing; however, it would obviously feel bad for me to see any of my family or friends die, so maybe just wishing that nothing bad to ever happen to me would already make none of them die, so the restriction to this wish would be that in fact it doesn't solve the problem of death. I guess wishing something like "no one to ever die" would not be possible, unless people would then travel to other planets, but if the universe isn't infinite, then even then it wouldn't work in the very long run; granted, wishing something like the universe to become infinite would solve that, but who would waste a wish on something like that, so I think that there just have to be restrictions).
3. There are many options to choose from regarding what to do with this last wish. Sparing it probably isn't a good idea, since my 2nd wish already stops anything bad from happening to me; in case it doesn't stop death, then this 3rd one would be me not ever dying. Otherwise, there can still be two problems. Not having a lot of money isn't exactly bad as long as someone has a decent amount of it [However, using the transforming ability, it would be very easy to get into anywhere and get as much money as I want.], and not finding the girl that would be the most perfect for me might not be categorizable into 'bad' either by the one who grants these wishes for me. Probably the latter problem would be one of the best choices here to fix using this last wish.
Getting to know a 100% solid future instantly leads to paradox, unless the person that gets to know the future loses his/her own free will and very likely his/her consciousness as well.
Getting to know how the world actually works, and/or finding out if God actually exists or not would probably be unfortunate as well, and oxymoronic in the first place if someone can grant me wishes that could change everything, so again, there most likely would be 'some' restrictions to those wishes if this would ever happen in reality.
Something interesting would be wishing for some sort of simulator (not sure how it would work, it could be an alternate dimension that you could enter, but if you would never leave it, would time freeze in the 'real' world forever?, so perhaps it might be best to say that all that could go down in one's mind, and the time passage speed in there could be adjusted to be of a certain portion of the 'real' world's time passage speed) where any place and a perfect copy of any characters who live/will live/have lived in the 'real' world could be 'summoned', and they'd act the same way in the situations that I'd think up as they'd act in the 'real' world, so that I would be able to get the best out of any situation [so this utility of it is basically almost the same thing as, but probably even more useful than, being able to read people's minds], not to mention how fun it could be to experiment with absurd situations, especially when combined with the transforming ability; and, more importantly, of course to hear/read/see already dead musicians'/writers'/painters'/etc. 'new' works.
[I didn't mention non-existing characters to be summonable in the simulator due to the fact that reactions are very complex, so non-existing characters would basically act the way I would think they'd act, and a wish isn't needed for that. However, perhaps if they wouldn't be in the center of attention, but you'd need them to initiate a certain situation with existing characters in it as well, then sure.]
The possibilities are endless, but only a very few of them are actually really useful compared to what a chance someone would get if this actually happened. Hell, someone could first wish to get to know the two absolute best wishes possible, and then use those up.

Yeah, I got really bored.
